Evaluation of Drug-related Problems and Quality of Life of Older Adults With Chronic Disease Through Home Medication Review by Community Pharmacist in Besut, Terengganu

Abstract

Introduction: The older adults face high susceptibility to drug-related problems (DRPs) due to physiological changes, multimorbidity, polypharmacy, cognitive shifts, and atypical presentations. This study aimed to assess DRP prevalence, health-related quality of life (HR-QoL), and the association between variables. Materials and methods: Data was collected in 76 older adult populations between January to March 2023 in Besut Terengganu by trained community pharmacists through the Home Medication Review (HMR) program. HMR protocol and EQ-5D-5L has been used to measure the outcome. DRPs have been classified into the group by using PCNE classification v9.1. The data was analyzed using Statistical Package for the Social Science Version 25 (IBM SPSS V25). Results: 76 older adults had enrolled in the study. A high prevalence of DRPs was observed, with 85.53% (n=65) of participants experiencing at least one identified DRP. 119 DRPs were identified during the study. According to PCNE classification, 50.00% of them are classified under code PI (treatment effectiveness). DRPs were significantly associated with polypharmacy and comorbidities (p=0.021). The mean visual analogue scale score is 66.63 (n=56) and the mean EQ-5D score is 11.66 (n=56). There is a significant association between HR-QoL and gender (p=0.039). The presence of DRP is significantly associated with multimorbidity (p=0.000461) and polypharmacy (p=0.021). Conclusion: A high prevalence of DRPs and a moderate HR-QoL were observed among the older adult population. The HR-QoL of older adults has a significant association with gender. The study shows older adults with multimorbidity, polypharmacy, and gender factors are vulnerable to DRPs.
