Incidence of Rheumatic Fever and Rheumatic Heart Disease and Its Associated Factors Among Paediatric Patients in Malaysia

Abstract

In impoverished and developing regions, acute rheumatic fever (ARF) remains a significant cause of morbidity and premature death. In Malaysia, particularly in the East Coast of Peninsular Malaysia and East Malaysia, ARF continues to be a leading cause of acquired cardiac disease. Despite this, there is a lack of published data on ARF incidence. This study aimed to determine the incidence of ARF and identify associated risk factors to enhance ARF prevention strategies. Materials and methods: The study, conducted over one year from July 2021 to June 2022 across 23 centres in Malaysia, included all newly diagnosed ARF and RHD cases, with a ratio of one case to two control patients. Sociodemographic and clinical data were collected, and multivariate analyses were conducted to identify associated risk factors. Results: The incidence of ARF was found to be 34.1 per 100,000 paediatric admissions, primarily concentrated in Kelantan and Sabah. Carditis (85.7%) and migrating polyarthritis (31.4%) were the most common clinical manifestations, with valve involvement, particularly mitral regurgitation, being the most frequent anomaly (97.1%). Multivariate analysis indicated that age, low income, and rural residence were significant risk factors for ARF. Conclusion: This study highlighted a persistently high incidence of acute rheumatic fever in Malaysia, with strong correlations observed between age, low socioeconomic status, and rural residence, and the likelihood of contracting ARF. These findings can inform more effective strategies for ARF prevention in the country.
